Conscious sedation events: reading the record after something goes wrong

Moderate (conscious) sedation is routine · until it isn't. When a sedation case goes wrong, the answer is usually in the monitoring record and the medication timeline.

Dosing and titration

The agents, doses, and timing matter. Stacking doses, inadequate spacing, or failing to account for a patient's risk profile can push a patient too deep.

Monitoring and rescue

Continuous monitoring, reversal-agent availability, and a documented response to oversedation are the safeguards. Their absence in the record is significant.
